3/24/2023 EnchantaVaughn Approved 1. Behavior: Client was present and very relaxed when QMHP arrived. Client greeted the QMHP in a friendly manner. QMHP engaged mother in a discussion about behaviors since the last session. Mother reports client struggles with getting along with her peers and showing increased aggression. 2. Behavior: Client waited for instructions, displaying polite and calm manners during the second half of the session. Client continues to reports issues with learning how to get along with her peers as sometimes they do things to her that she does not like such as pick on her. 1. Intervention: QMHP explored positive behavior options with client to support her willingness to make good decisions in the home. QMHP engaged in conversation with client about positive decisions and her plan to continue building relationships with her mom and sibling. QMHP inquired about evening routine and after school activities client is currently involved in. QMHP assisted client in monitoring her own behaviors in efforts to promote exercising self-control in challenging areas. 2. Intervention: QMHP reiterated the expectations and rules for the session. QMHP applauded client for wanting to work on changing her behavior so that she leads by example and takes responsibility for her actions. QMHP reviewed client after school schedule with her and supported as she prepped for the evening work she had. QMHP commended client on utilizing creative strategies to increase her understanding of feelings and emotions as it relates to her mental illness. 1. Response: Client responds well to praise and support. Client implemented good communication skills by listening attentively. Client verbalized good thoughts and understanding. Client developed better communication skills by expressing herself in a calm manner. Client displayed management of individualized behaviors. Client has not overreacted with anger to minor frustrations or irritants. 2. Response: Client expressed positive self-talk, modeled positive behavior, and good communication skills during role play activity. Client is making progress with expressing her feelings and showing others respect and courtesy by talking with appropriate tone, initiating conversation, and waiting for her turn to talk. Client responded well to feedback. Client communicated a positive attitude when reviewing problem solving and self-control strategies with her support system.
